I give permission for my young person to participate in all events at the National Youth Rally held at Beeches Road Baptist Chapel. I also give permission for my young person to be transported to Flip Out Trampoline Arena in Oldbury and to participate in the activity there.

I give permission for the CCHT and its affiliated ministries (ie, Camp Victory) to take photographs of my young person whilst at the NYR and understand that the photographs will not be sold in any way, but may be used in promotional materials or added to albums on the website and FB pages for viewing at a later date.

The event is held at Beeches Road Baptist Chapel, Rowley Regis B65 0AT.

Transportation will need to be provided via the family of the young person or the associated church. Crown Christian Heritage Trust is unable to supply transportation to Beeches Road Baptist Chapel.
